You are invited to participate in a brief survey for Pediatric Congenital Heart Defect (CHD) research.

I, Ashlee Loughan, am a doctoral student recruiting participants for a study titled: Late Effects of Congenital Heart Defects on Parent Ratings of Executive Functioning Skills in School Age Children. I am looking for parents of children with CHD who are between the ages 5-18.

Specific Congenital Heart Defects I am researching are:

Double Outlet Right Ventricle Truncus Arteriosus
Single Ventricle
Total Anomalous Pulmonary Venous Connection
Critical Coarc
Critical Pulmonic Stenosis
Pulmonic Stenosis
Atrioventricular Septal Defect
Aortic Stenosis
Ventricular Septal Defect
Patent Ductus Arteriosus
